Chapel View Family Care is dedicated to providing you with medical care that is both cost effective and of the highest quality. All deductibles, co-pays, and co-insurances are due at the time of visit. Alternatively, some patients find it convenient to set up an automatic bill-pay account with their credit card.

Following each visit, the patient's insurance company determines the cost of the visit based on the level of benefits covered by the patient's insurance plan. For questions regarding the determination of costs, please contact your individual insurance company about the coverage specifications of your policy.​
